According to a report from the Australian Transport Safety Bureau (ATSB), an unrecorded modification to a steam trap line on the tanker “Maritime Wisdom” caused a serious incident resulting in injuries to three crew members.
The incident occurred on May 6, 2025, while the vessel was anchored near Sydney. An engineering team was performing maintenance on a steam valve of the heating system on the main deck when they encountered an unexpected problem.
After isolating the system and removing the valve cover, hot condensate was unexpectedly released, scalding three crew members. They received emergency first aid on board before being transferred ashore for further treatment.
The ATSB investigation indicated that sufficient cooling time was not allowed before work commenced on this critical component of the cargo heating system. Furthermore, it was found that the steam trap line had been permanently altered without any formal documentation or risk assessment.
Chief Commissioner Angus Mitchell stated, “This unrecorded modification likely created a flaw in the isolation process of the steam system.” He emphasized that such modifications should undergo comprehensive review and be documented as part of standard safety procedures.
The investigation also highlighted oversights during routine inspections; these unauthorized alterations were repeatedly missed by technical teams and company superintendents who had conducted multiple inspections on the tanker “Maritime Wisdom”.
Mr. Mitchell added, “Failure to follow our management of change framework meant the necessary risk assessment was not conducted.” “This incident underscores the importance of operators adhering to established safety procedures.”
In response to the incident, the management of the tanker “Maritime Wisdom” has restored the drain line to its original design and initiated an action across its entire fleet to identify any unauthorized modifications to piping systems. Additionally, shipboard personnel will be reminded to communicate with management before making any alterations, in accordance with company policy.
Mr. Mitchell concluded, “Operators must ensure adequate cooling time and visually confirm correct isolation during maintenance involving steam systems.”
