A tragic incident unfolded on October 14, 2023, aboard the U.S.-flagged offshore supply vessel Red Stag in Port Fourchon, Louisiana.A mariner lost their life due to electrocution while trying to fix a mud pump system. The unfortunate events began around 5:30 AM when the victim inquired about the location of a voltage meter from a deckhand. Just over an hour later, at approximately 6:40 AM, the vessel’s Master found them unresponsive next to an open high-voltage panel in the engine room. Despite swift emergency efforts, they were declared dead by 7:40 AM.

Commander J.E. Fothergill from the U.S. coast Guard expressed deep sorrow over this preventable tragedy. An examination revealed that several factors contributed to this fatal accident; primarily, it was linked to a suspected material failure within the mud pump system itself.The inquiry uncovered meaningful safety lapses that played crucial roles in this incident. Notably, it was found that the victim lacked proper qualifications and did not adhere to established safety protocols. There were also no visual or audible signals for activating emergency stops and missing locking devices on high-voltage panels.

The sequence of events started during a routine watch change at 11:30 PM when the outgoing first engineer informed what is referred to as “Unqualified Engineer (UE)” about leveling operations using the mud pump system. The incoming crew’s responsibilities included cleaning up in the engine room and preparing for mooring stations.

While Job Safety Analyses (JSAs) had been completed for their tasks, attempts made afterward regarding troubleshooting lead directly to this fatality—highlighting how multiple layers of safety failed and allowed an unqualified person access energized electrical equipment.

In light of this tragedy, adriatic Marine LLC—the owner and operator of Red Stag—initiated a fleet-wide Safety Stand-down aimed at reinforcing dialog between captains and crews while stressing adherence to safety policies.The investigation concluded without any findings suggesting criminal wrongdoing or legal violations by Coast guard personnel or certified mariners involved; however, it serves as a critical reminder about ensuring proper training and strict compliance with safety measures within maritime operations—especially concerning high-voltage systems.
