According to a report from the Transportation Safety Board of Canada (TSB), there is an urgent need for improved safety protocols on ships following a serious incident involving the bulk carrier Golden Zhejiang in 2023.
The TSB highlighted that this event underscores the necessity for more defined procedures regarding risk assessments, specialized training for equipment use, and a robust safety management system. A culture of safety and effective communication between crew members and shore personnel are also critical components that require attention.
The incident occurred on September 2, 2023, while the vessel was anchored in British Columbia’s Trincomali Channel. During a routine weekly inspection of the free-fall lifeboat, two engineers were performing checks when an unexpected situation arose.
While testing steering mechanisms inside the lifeboat, one engineer inadvertently triggered its release mechanism. This led to a rapid descent into the water with him still aboard since he wasn’t secured in his seat at that moment-resulting in significant injuries.
The investigation revealed gaps within the ship’s safety management framework; specifically, it did not mandate risk assessments prior to entering lifeboats for inspections. Consequently, these engineers proceeded without considering potential hazards associated with accidental releases. Furthermore, it was found that securing cables used were neither engineered nor certified as load-bearing-rendering them inadequate for their intended purpose.
In response to this alarming event, Columbia Shipmanagement Ltd., which operates Golden Zhejiang, has implemented several key safety improvements. These enhancements include clearer labeling on lifeboats and updated checklists along with work instructions aimed at better familiarizing crew members with equipment operations. Additionally, securing turnbuckles have been installed to bolster safety measures.
This incident has also been integrated into ongoing crew training programs while shore management has increased its visits to vessels to enhance awareness around free-fall lifeboat protocols among crews.
The TSB emphasizes that establishing clearer guidelines and stronger safeguards is essential in preventing similar incidents during standard lifeboat inspections moving forward.
